Nursing facilities (NF) are institutions certified by a state to offer 24-hour medical and skilled nursing care, rehabilitation, or health-related services to individuals.
Medicaid helps cover three different levels of care including 1) Skilled Nursing Care 2) Intermediate Nursing Care and 3) Residential Care.
Case Management Agencies assist with referrals to nursing facilities.

To qualify:
- Be 65 years or older and/or meet other targeting criteria
- Meet the functional and level of care requirements which are determined through a designated assessment process.
- Meet financial determination requirements as outlined in the Colorado Code of Regulations and as determined by your local Department of Human Services.
