Case Management

If you’re new to the I/DD realm, you have probably noticed we use various acronyms. To help you navigate all of them, we’ve created a guide of frequently used acronyms and terms used by specific departments or programs. Today we’re sharing the Case Management portion of the guide.

The Case Management Department is responsible for determining eligibility and managing ongoing care for children and adults with I/DD seeking or receiving services through Home and Community Based Service Medicaid Waiver Programs (CES, CHRP, DD, SLS), or State-funded programs (SSLS). It’s a big team made up of hardworking, compassionate case managers who navigate the complex system of I/DD care so that you don’t have to. Read on to learn more about the lingo you might encounter when seeking case management services.
